I used HPUSBFW, a free software by HP to format USB drives, which also works on SD cards. Takes a long time, but worked flawlessly on my 64GB card. Must be run as administrator.

I used a 256 GB variant formatted as FAT32 on macOS as outlined at
http://forum.krontech.ca/index.php?topic=34.msg568#msg568
It seems to work just fine!
